It is our policy to provide our patients with the best available care and facilities at a reasonable cost. For the convenience of our patients, we participate in many insurance plans including various HMOs and commercial insurance plans. In an effort to eliminate the expense of billing and collection, thereby keeping your cost down, we ask that you pay for services which are not covered by insurance at the time of the office visits. These may include the cost of the office visits, orthopaedic appliances, co-payments and deductibles. In most cases x-rays, fracture charges and casts can be submitted to the insurance company.

Except in certain circumstances, your health insurance is a contract between you and your insurance company. It is necessary, therefore, that you take the responsibility for the follow-up of a claim with your company if you feel it is not being paid promptly or properly. We will be happy to provide you with whatever information we can to help you substantiate your claim. However, the responsibility for payment of your bill is yours. In order for us to file an insurance claim, it is necessary that we be provided with all the pertinent information including policy holder's name, contract number, group number, claim number and policy number. In cases of workman's compensation, we need the name of your employer as well as the name of his workman's compensation insurance carrier. In compensation cases it is always easier if we have the name of the person who is handling the claim.

This office does participate in the Medicare program. Therefore, Medicare claims will be processed and sent by the office.

It is important that you realize, however, that Federal law requires that patients be balance-billed for the twenty percent (20%) of the allowed charges which Medicare does not pay. Therefore, patients covered by Medicare can expect to receive a bill for this balance.

For the convenience of our patients, we also accept MasterCard and Visa credit cards.

We recognize that very often financial hardship may be a problem, especially in the case of prolonged illness, unemployment or other unusual circumstances. In these cases, some type of special consideration may be extended.

It is important that you discuss your financial problems early in the course of your treatment in order to avoid any misunderstandings which may occur.
